Agency Agreement

All chapters wishing to affiliate with Zete Kids U.S.A. must fill out an agency form allowing them to act as an agent for Zete Kids. This is beneficial to chapters, as you can then collect donations on behalf of Zete Kids U.S.A. - a 501(c)3 organization. This means those who donate during your event could potentially make a tax-deductible donation.

You must fill out this agency agreement once.
